Per Diem Weekend Admission Nurse
Hospice case managers support and counsel clients who are facing terminal diseases and conditions. They typically coordinate a collaborative hospice care team that may include healthcare providers, at-home care services, mental health specialists and spiritual advisors.Hospice Admission Nurse primarily functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assume the responsibility for coordination of care.
- Education: Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program. Bachelors of Science in Nursing, preferred.
- Licenses/Certification: Current state license as a Registered Nurse. CHPN certification, preferred for hospice locations. Proof of current CPR and Hepatitis profile. Current driver’s license.
- Must have reliable transportation and insurance.
- Experience: Minimum of 1-2 years experience as a Registered Nurse in a clinical care setting, home health and/or hospice preferred.
