Job Description

-Coordination of patient care with referral source and clinical staff

-Knowledge of HMO authorization process

-Act as a liaison to facilitate communication between patients/clients and the health care delivery agencies, and establishing accurate records.

-Inputting patient demographics into the system

Requirements

1 year Home Health experience preferred

Good communication skills

Good computer skills

Detail oriented self starter

Self-directed and able to work with little supervision

Familiarity with medical terminology a plus
